CBT Self-referral

Under the IAPT scheme (improving access to psychological therapies), we are now able to offer self-referral to Cognitive Behavioural Therapy. This has been shown to be very effective for the treatment of mile to moderate anxiety and depression.

You may wish to discuss this with your GP first, or they may have directed you to our website.

You can download the forms then complete and send them to the CBT department yourself, hopefully giving you quicker access to the help you need.

First Contact Physiotherapy Practitioner

First Contact Physiotherapists (FCP) will provide expert assessment, diagnosis and management of soft tissue, muscle and joint conditions.  A patient can book an appointment to see an FCP without having to see their GP first. 

You can book an appointment with an FCP by contacting reception. Please note, this service is for those aged 18 and over

Following an assessment the FCP will provide self-management and exercise advice, refer on or request further investigations if needed and offer joint injections in line with evidenced based practice.

The service aims to:

  • Provide a specialist expert assessment and guidance on self-management.
  • Achieve early intervention of care to promote health and return to work.
  • Manage an individual’s journey to other services.
  • Provide improved healthcare choice and satisfaction for residents of Stockport

We now have a physio on site on a Wednesday and Friday Morning, these physios can then help patients with exercises, advice and guidance. Very often this early intervention and support will stop patients having to go onto the physio service waiting list.  We do stress this is in addition to GP and physio services not instead off, so if a patient needs to see a clinician or physio they will continue to do so.

NHS e-Referral Tracking

If your doctor has referred you to a specialist for further care you will be asked to book your appointment using the ‘NHS e-Referral’ system.

You will receive a letter from us within 2 weeks giving details of how to book your appointment and any passwords you may need.

To track your e-Referral you will need the following details to log into the e-Referral website:

  • Your date of birth
  • Reference number – this will be on the first page of your letter
  • Password – this will be on the second page of your letter
