Wellbeing Service

We are now hosting a service where patients can be referred into that doesn’t always need a clinical intervention, things like support for carers, patients with anxiety, social prescribing, and lifestyle services.

This service is to help patients connect to more localised services and offer additional support.  We do stress that this is in addition to GP services and not instead off so if a patient needs to see a clinician they will continue to do so.

People will be supported to take control of their own health and focus on changing what matters to them. Supporting people living with a long-term condition requires a partnership with patients over the longer term rather than providing single, unconnected “episodes” of care. Using Patient Activation tools the service will appropriately match services to people depending on their level of confidence and motivation to change their behaviours. The service will offer five evidence based interventions which have significant potential to improve quality of life for people with long-term conditions and deliver benefits across the three dimensions of value: Mental and Physical health and wellbeing, NHS sustainability and wider social outcomes.

These are:

  1. Peer support
  2. Self-management education
  3. Health coaching
  4. Group activities to support health and wellbeing
  5. Asset-based approaches in a health and wellbeing context

Key benefits of this service are:

  • Support to General Practice releasing capacity in an already stretched service.
  • Patients underlying problems are appropriately identified.
  • Improved and appropriate care navigation within the health and social care system for the neighbourhood population, i.e. patients referred to the appropriate services based on their needs.
  • Improving access to the community and voluntary sector services and resources available to support people better value for all, i.e. more people can access the service with outcomes that will have a positive impact on both physical and mental well-being.
  • Facilitating access to informative and motivational online resources, including space for online mutual support
  • Proactive engagement and support for people to improve their self-care, tailored according to need using the Patient Activation Measure.
  • Increasing numbers of people actively engaging in voluntary activity, complementing and adding value to the work of Stockport Together to improve health, wellbeing and interdependence.
