New Patient Information

We ask all new patients over the age of 16 to attend the practice for a new patient check with one of our Health Care Assistants as this gives you the opportunity to be put on our register if you have any of the following - heart disease/diabetes/asthma/stroke or kidney disease - if this is the case we will then invite you in to the practice for a review.

If you have regular medication, prescribed by the doctor, hospital or chemist (including contraceptive pill and Depo-Provera injection) please provide a copy of your latest repeat medication form.

As in the message you may have already received from us, we have already registered you with a GP at our practice - this GP has overall responsibility for the care and support that the practice provides to you. Although you will be registered with a GP this does not prevent you from seeing any GP in the practice.

If you have indicated that you would like to register for online appointments and scripts - the registration documents will be sent separately via e-mail in approximately two weeks. If you have not registered for text messaging, online appointments or online prescriptions please feel free to ask for details or click the following link to continue with the application online http://www.gatleymedicalcentre.co.uk/doitonline.aspx
