Medical request forms are available Monday-Friday, 8am-2pm.
The submitted forms will only be read during office hours. The forms will not be read on the weekends (Saturdays and Sundays) or on bank holidays. They will not be read Monday-Friday between the hours of 18:00 – 08:00. Please contact 111 during out of hours or 999 if this is an emergency.
PRACTICE INFORMATION
Due to the increase in respiratory illnesses the practice have made the decision to ask all clinicians within appointments to wear a mask and all staff when moving round the building to again wear a mask, we also ask that all patients to do so in particular when they are attending for a respiratory issue.
